CASE MANAGEMENT
Case Management is an authorized service which helps clients and their families make informed decisions based on client's needs, abilities, resources, and personal preferences. Case management services can also personalize care in an otherwise impersonal system. (Ref: State of Florida 2001 HIV/AIDS Case Management Standards & Guidelines)
AHI Case Managers are certified after meeting rigorous criteria based on education, training, and background checks. Your Case Manager will contact you periodically in order to update your individualized service plan..
This helps them in coordinating your needs with your medical providers, and, in providing client advocacy and crisis intervention.
After regular business hours (Monday-Friday, 9AM - 5PM) you can reach a Case Manager for emergency needs, which require immediate response, by calling 305-304-3446.
