Every AIDS Help client has the responsibility to take part in developing their own Individual Plan of Care.
Your case manger is a well-trained professional familiar with HIV and opportunistic infections associated with the virus and AIDS. Each year your case manager receives updates on the basic HIV-500 training and he or she attends seminars, training, and conferences to ensure they remain well informed.
However, even the best-trained case manager is unable to guide you in a Plan of Care if he/she does not know the complete story about your past illnesses, problems, and treatments. Only you have the information that is so important in developing a plan to ensure access to the best quality of care available in Monroe County.
Let your case manager know about your physicians, insurance carrier, your place of employment, alternative treatments, medications, your home and family, your partner, and the type of problems you face in your daily life. But always remember, that you have the right to provide as much input as you want, and approve or reject elements of the Plan of Care that is developed.
